Holiday Absence Request Form

The holiday request absence for must be completed 3 weeks before the first day of the requested holiday. All holidays (regardless of whether it is likely they will unauthorised or not) must be registered with the school. In the event of the Local Authority issuing you with a fine, the information that you submitted (detailing the reasons for taking holiday during term time) could be used to support your appeal against the fine.

IMPORTANT NOTICE

Before completing this form, may we respectfully remind you that Rushmere Hall Primary School does not authorise holidays during term time and will also not authorise leave for a celebration of any description. We also do not authorise lateness to school.

We will track absences carefully and where necessary the school will refer cases to the Education Welfare Services.

    Please provide information of other siblings within this school...
    I, being the Parent of the above Child/Children, request that you consider a “leave of absence” from school for the following exceptional circumstances.​
    ​(for Guidance, please information below) 
    I understand that your decision will be taken after due consideration of:
     Previous history of attendance (including that of a previous school)
    I have read the guidance notes and understand that permission for a family holiday will not be authorised and understand that my actions are likely to incur a Penalty Notice from the Local Authority.

    Guidance on requesting Leave of Absence

    1. Rushmere Hall Primary School will consider every application individually. Its policy is NOT to grant “leave of absence”, other than in the most exceptional circumstances. Time off school for family holidays or celebrations is not a right. An application must be made in writing, with appropriate evidence and in advance.

    2. Leave of absence is defined as an absence other than an annual family holiday, wedding, birthday or celebration event. Such an event may be an unfortunate need to attend a funeral, or to attend a child’s pre-arranged hospital appointment (but not a regular appointment at your GP, Optician or Dentist).

    3. Rushmere Hall Primary School will consider AUTHORISING “leave of absence” for;
     Armed Forces personnel returning from active duty
     a Family crisis inc. a sick relative* 
     Funerals*, Weddings*
     A pre-arranged hospital appointment that cannot be changed
     Religious observance of a Nationally or Internationally recognised occasion
    *Please additional guidance on this included in the policy. 

    4. Requests for “leave of absence” will be deemed UNAUTHORISED for;
     A family holiday, regardless of a parent’s employment restrictions
     Family celebrations (e.g. Birthdays)
     A regular GP, Optician or Dentist appointment


    5. Rushmere Hall Primary School will respond in writing to all requests to take a child out of school

    6. Rushmere Hall Primary School will NOT authorise family holiday leave during term time.

    Important : The Local Authority Education Welfare Services will actively seek to track and take action where necessary for those parents who persist in taking their child/ children on holiday during term time. In such cases the Local Authority will notify the parent concerned and issue a Penalty Notice where necessary. We urge all parents to be aware of this when making your decision.
