Pick Up Arrangement Forms

    AMBER FORM

    If you have already given permission for your child to have an amber form, you do not have to complete this form again.
    COMPLETE THIS FORM FOR YOUR CHILD TO HAVE AN AMBER CARD – ideal for children meeting parents in different playgrounds
    • The default arrangement is that children are NOT allowed to leave school by themselves.
    • Complete and return the form to your child’s class teacher. Copies of this form are available from reception.
    • Children will be provided with an AMBER card to show they have permission to leave their class teacher, navigate their way around the school site BUT not leave at any of the exit gates.

    GREEN FORM

    If you have already given permission for your child to leave school by themselves, you do not have to complete this form again.
    Ideal for parents who are allowing their children to walk home from school by themselves. 
    • Please provide us with confirmation if you are allowing your child to leave school by themselves.  
    • The default arrangement is that children are NOT allowed to leave school by themselves.
    • Complete and return the form to your child’s class teacher. Copies of this form are available from reception.
    • Children will be provided with a GREEN card to show they have permission to leave school by themselves at the end of the school day.

    Older Sibling Pick Up Form 

    Please complete this form if there will be occassions that an older sibling (of school age) is expected to pick up your child from school. 
    When on the school premises, the sibling must behave sensibly and be respectful of other parents and school staff. They must not use bikes or scooters on the playground and they must not be with a group of friends.
